Your First Visit

Getting Started

Before we can schedule your first visit at Samaritan Counseling Center, please complete and submit the New Client Packet. The information you provide helps us to match you with a counselor that will best meet your needs.

Don’t have a printer? You can pick up a paper copy at our office.

Before your session

Please bring the following items:

• Photo ID, one of the following:
  • Driver’s license
  • College ID with photo
  • Workplace ID with photo
• Health Insurance Card (we accept many different insurance plans)
• Co-payment
• Minors under 18 must be accompanied by their parent or guardian to sign paperwork.

Cancellation / Rescheduling

If you need to cancel or reschedule your first visit, please provide 24 hours notice. If you are unable to attend two scheduled initial appointments for any reason, you may be placed back on the waiting list.

People Under 18

The New Client Packet must be completed by a parent or other legal guardian for any person under 18. We also ask that a parent or legal guardian remain present on the property for the duration of all future appointments.

Where To Go

Please come to our office location. Please head into the main office (the brick building). Our office staff will help with any paperwork, and notify your counselor of your arrival.
